UK Dentist: NHS treatment, do I pay if I’m on income support?

I’m with an NHS dentist in UK, am currently on income support & have to pay NHS costs each time I visit. I have been told must have check-ups each year to remain registered, which I have to pay for & I see the price keeps going up & up. Have been told by a support worker I can claim back the cost of check-up/essential treatment if I send off a receipt + benefit claim form within 3 months. Would it make any difference if I let dentist know I was on income support? I hate asking for a receipt & also never get a claim form in time, so can’t send off & never get the money back.

Your dentist’s receptionist can give you the form, and a receipt, but they can’t fill it in for you.
The form asks specific questions about your financial circumstances, and what benefits you receive.
Without this form, the dentist can’t give you free care.
P.S. You can backdate claims up to 3 months… that should be more than enough time to send off the receipt and form.
